Our Medicaid Service Coordination Department (MSC) is a Medicaid State Plan service administered by the NYS Office of Mental Retardation and Developmental Disabilities (OMRDD). MSC assists people with developmental disabilities in gaining access to necessary services and supports to lead successful lives in the community according to their own needs.
The Family Care Department provides individuals with developmental disabilities an opportunity to live with a family in the community. This is open to individuals of all ages. In addition, Family Care providers receive training and ongoing support from professional staff to assist them in meeting the needs of individuals in their homes. MHS currently offers Family Care Services throughout Mid Hudson Valley counties.
The Day Hab Program is designed to meet the needs of a wide variety of persons with disabilities. MHS provides services in such areas communication, socialization, interpersonal relations, civic awareness, environment, health and safety, transportation, recreation, leisure, and community knowledge.
The In-Home Residential Habilitation program provides services to Medicaid Waivered enrolled individuals who reside with family members or independently in the community. This service provides the opportunity for the individuals to further /advance their skills in the areas of mobility, social behaviors, self care, basic safety, housekeeping, personal hygiene, health care and financial management. This service is provided by a Residential Habilitation worker who is assigned to the individual and works one on one on a regularly scheduled basis. An individual who wishes to receive At Home Residential Habilitation is referred by a Medicaid Service Coordinator.
The After School Program is specifically designed to meet the needs of school aged children with developmental disabilities. Each afternoon, children participate in educational, recreational and socialization experiences designed to enhance academic and social skills while boosting self-esteem. Homework help will also be available. The program, designed and supervised by professional educators and clinicians, offers the opportunity for children to enjoy a safe, supportive and successful after school experience.
The Family Support Program provides individual services and supports to families that include a child with severe disabilities. The Program recognizes that meeting the needs of children who have severe disabilities may place hardships on a family's emotional, physical, and financial resources.
Provides and coordinates health care and professional nursing services for individuals in our family care program.
Someone who finds services for persons with special needs and sees that they are coordinated.
